Training Request Form
I created the Training Request form to act as a preliminary needs analysis that a stakeholder would complete when submitting a training request at DUOS. The questions they responded to gave me some key information I could review before our project kickoff meeting. A new form submission was set to automatically email me a notification so that nothing was missed.
